History & meaning
Makoto runs the opposite way from most Japanese names. With Kenji or Kaito the sound is an empty vessel and the characters pour the meaning in. Here the word came first. Makoto is plain Japanese vocabulary — sincerity, good faith, the truth of a thing as against its appearance — and it turns up in classical prose centuries before anybody used it on a birth register. The kanji are simply competing ways of writing a word that already meant something. 誠 wins most of the time, 真 and 信 and 実 take the rest, and none of them changes what a parent is saying.
The character has a second life as a slogan. The Shinsengumi, the swordsmen the Tokugawa authorities used to police Kyoto in the 1860s, marched under a banner carrying nothing but 誠 — one character, one word, sincerity, meaning something closer to unswerving loyalty in that context. It is still the image most Japanese people picture if you write the character on its own, and it gives the name a flint that the English gloss "sincere" does not quite convey.
Why parents choose it
Two reasons that rarely coincide. The meaning is fixed, which almost no Japanese name of this length can promise, and the name is genuinely unisex in Japan rather than merely borrowable. Add three transparent syllables and an open final vowel that puts it in company with Kenzo and Hiro on any list of names ending in O, and it travels well.

Famous Makotos
Makoto Shinkai directed Your Name, the 2016 film that briefly outgrossed every anime except Spirited Away. Makoto Hasebe captained Japan at the 2014 and 2018 World Cups and went on playing in the Bundesliga into his forties. Makoto Kobayashi shared the 2008 Nobel Prize in Physics for work on the origin of broken symmetry in the quark sector. All three write it 誠.
Consider instead
Akira is the other great Japanese unisex word-name, brighter in sense and better known abroad. Nagi is the same idea in half the length — a plain noun, used for both sexes. Satoshi keeps the three-beat shape and the intellectual register.
